Can I keep my personal physician?

Yes. Unlike PACE/LIFE programs that require use of their healthcare provider, we will work closely with your personal healthcare provider and help reinforce the primary physician/patient relationship. We consider this bond to be a high priority.

Who is eligible for Adult Day Services?

Any adult over the age of 18 who is unable to independently perform self-care activities or function alone safely.

What are the hours?

All of our Adult Day Centers are open Monday – Friday, 7:30 a.m. to 5:00 p.m. We are closed on major holidays.

Who pays for Adult Day Services?

We accept Self Pay as well as funding sources including Medicaid Waiver, Long Term Care Insurance, VA, as well as various state and county programs. Charitable care can also be provided to individuals with limited resources.

Are Home Nursing Agency centers licensed?

Yes. All centers are licensed through the Department of Aging.

Who can make a referral?

Anyone can make a referral. Simply call us at 1-800-445-6262 to find out what we can offer for you or your family member. Your physician or other referring agency may also make recommendations regarding your healthcare – just be sure to ask for Home Nursing Agency.
